How do I switch my plan management to Provider Choice?

Switching to Provider Choice takes three steps and causes no gap in your supports:

  1. Email your current plan manager: "Hi [name], I'd like to end my plan management services with you. Please confirm my plan release date." They'll confirm the date your funding is released.
  2. Sign up with us online: it takes about 60 seconds through our online app. We'll handle the setup and confirm your start date.
  3. If your plan is in the PACE system, endorse us with a quick call to the NDIA: our provider number is 4050041521 and we'll give you a simple script for the call.

Your providers keep getting paid, your supports continue and you start seeing your budget in the real-time dashboard from day one of having you set up.
