I'm writing to let you know I'd like to end my plan management services with you. I'll be moving to Provider Choice.
Action Required:
Please confirm the date my services will end and when any remaining funds will be released to my new plan manager.
Please ensure to CC Provider Choice into your reply: planmanagement@providerchoice.com.au.
Authorisations:
- I authorise Provider Choice to contact you on my behalf to arrange the transition. This includes releasing my existing service bookings and retrieving my latest activity statement.
- If any invoices come through that you're unable to process, I authorise you to forward them to Provider Choice on invoices@providerchoice.com.au for processing.
- If any of my providers contact you about my plan, please let them know I've moved my plan management to Provider Choice and share Provider Choice's contact details with them (see above).
