Can someone help me with my application?

If you contact our team at Provider Choice at team@providerchoice.com.au, then we can put you in touch with your local area coordinator (LAC) who can help you with the NDIS application process.

What sort of paperwork does the NDIS planner need?

Take in assessments and reports (where relevant)

  • Diagnosis from specialists
  • Home modification assessment
  • Medications charts
  • Mental health reports
  • Care needs assessments
  • Occupational therapist and speech pathologist reports
  • Physical condition/accessibility needs

Think about what assistive technology and equipment you needs, remember it must be ‘reasonable and necessary’, take in quotes for this equipment and any reports from school or professionals verifying that this is what he needs.

Always remember you can submit more documentation just after your planning meeting whether that be specialist reports or quotes for products.

What are goals?

A goal is something that you want to do. Goals can be short term, so something in the next 3 months, or long term, so looking at the next 5–10 years.

Your NDIS plan will contain a number of goals that you wish to achieve. We can help you to set goals for yourself and these go into your individual support plan. Then we can help you work out what supports you need to achieve your goals. How do I build a goal?

To plan your goals, start by thinking about what you'd like to achieve. That might be things like joining in with more social activities or increasing your physical strength or maintaining your independence.

Our pre-planning tool can help you build your goals which you can find here, otherwise you can get in touch with us by emailing hello@providerchoice.com.au. We can talk you through your goals and to work out the supports you need to make the goals happen.

How do I put my plan together?

Before meeting with your planner you should think about what supports you need, and what goals you want to reach. You can start with our preplanning tool which is here or by calling us and we can help you put together a draft plan to help with your planning meeting. You will then work with an NDIS planner to make a plan that suits your needs and your goals. You can contact us and we can help you with this if you email hello@providerchoice.com.au.

If there is more than one child in a family with a disability, does that affect the support we can get?

If you have more than one child with a disability, you'll have a separate plan for each child. This is because each person is different. This means most supports will be separate but on certain occasions the NDIS might also combine some when it makes sense to.
