Switching plan managers is simple! Here’s how to do it:
Let your current plan manager know in writing Send them an email letting them know you’d like to end their services. You can use a simple template like: “Hi [Plan Manager], I’d like to end my plan management services with you. Please confirm my plan release date.” They’ll reply with your release date. That’s when your new plan manager can start.
Contact your new plan manager to sign up Once you’ve chosen your new provider, they’ll help you complete a few forms and confirm your start date. If you’re joining Provider Choice, we’ll walk you through every step and make the process smooth and stress-free.
(PACE only) Call the NDIA to ‘endorse’ your new plan manager This step only applies if your plan is in the new PACE system. Provider Choice is PACE-ready, so we’ll help you check if this applies to you and explain exactly what to do.
