Once you've chosen who will provide your supports you may want to create a written service agreement. This will help make sure that you and your provider both know exactly how your supports will be given to you.

The key things to think about are:

• how much will it cost? (n.b. if you agency or plan manage they will need to charge at or under the price cap)
• what date does it start?
• how often will i have it?
• what date does it end?
• what happens if I need to cancel? (do fees apply?)
• how do we resolve issues if they arise?
