Fast, Paperless Consent for Participants & Nominees

At Provider Choice we’re constantly looking for ways to remove friction for participants and families. We understand less admin means more time caring for each other. That’s why we’ve updated our consent process to allow you to take care of consents quickly and securely online. 

No printing, scanning or paperwork needed. 

Introducing Digital Consent for participants and nominees

It only takes a few minutes inside your Portal.

  1. Log in to your Provider Choice Portal and head to the Permissions tab.
  2. Click Invite Someone New and enter your trusted person’s details.
  3. Choose the level of consent you’d like to give - we explain your options below.
  4. Click Invite and you’re done.

Your trusted person will receive an email invite. Once they accept it, we’ll be notified automatically and their details will be added to your profile.

Levels of consent

‘Share’ Consent (recommended)

This is the standard and most commonly used option.

It allows us to share the information your trusted person needs when they contact us for support, without giving them control over your consent settings.

If this is what you’d like, simply leave the Manage checkbox unticked.

‘Manage’ Consent (only if necessary)

This option goes a step further. It allows your trusted person to give or remove consent for others on your behalf.

Think of it like giving someone a key to your house and allowing them to make copies of that key. For that reason, we recommend keeping this level of control for yourself unless there’s a clear need to share it.

If you want to provide this level of access, tick the Manage checkbox.

What is ‘Consent to Share Information’?

Giving consent means you’re allowing us to share relevant information (including confidential information) with someone you trust.

This helps them support you to:

  • Understand your NDIS plan
  • Support funding decisions
  • Assist you to navigate your NDIS journey

It’s always your choice. You stay in control of who can access your information and what they can see.

Who might you give consent to?

Many participants and nominees choose to provide consent to:

  • Support Coordinators: They often help organise services and support funding decisions.
  • A trusted family member or friend: If someone regularly helps you manage your supports or payments, it can make sense for them to be able to speak with us on your behalf.

We recommend only giving consent to people who genuinely need it to support you.

What information might be shared?

We only share information that is relevant and necessary to provide our services. This may include your personal and contact details, NDIS plan information and funding levels, invoice details and information about the providers and services you have engaged.

A quick note: we don’t recommend giving consent directly to providers. In almost all cases, we can support your providers and answer their questions without sharing any of your confidential information.

Want to change or withdraw consent?

You can update your permissions at any time through your portal. In the Permissions tab, you’ll be able to see who currently has access to your information, review any pending invites you’ve sent and revoke access if things change.
