Once you've checked your eligibility, you'll need to complete an access request form provided by the National Disability Insurance Agency (NDIA). This form collects essential information about the applicant's disability, its impact on daily life, and existing supports. Once submitted, the NDIA evaluates the application to determine eligibility for the NDIS. This process may involve further assessments or additional documentation from healthcare professionals.
